I had incredible problems with customer service with this company. I only visited 3 times and they were given my insurance information. And, I paid my co-pay's in advance.

Almost a year later I was served with a small claim stating the insurance never paid.

I contacted every office they are affiliated with and was unable to speak with anyone in charge. I was "mammed" to death" by their staff who for some reason were "hiding" the contact information for anyone with authority. They would not give out any information ...not even an email address!
