[b]Mechanism of action


As an antifungal, ketoconazole is structurally similar to imidazole, and interferes with the fungal synthesis of ergosterol, a constituent of fungal cell membranes, as well as certain enzymes. As with all azole antifungal agents, ketoconazole works principally by inhibiting the enzyme cytochrome P450 14-alpha-demethylase (P45014DM). This enzyme participates in the sterolbiosynthesis pathway that leads from lanosterol to ergosterol. Lower doses of fluconazole and itraconazole are required to kill fungi compared to ketoconazole, as they have been found to have a greater affinity for fungal cell membranes.

As an antiandrogen, ketoconazole operates through at least two mechanisms of action. First, and most notably, high oral doses of ketoconazole (e.g. 400 mg three times per day) block both testicular and adrenal androgen biosynthesis, leading to a reduction in circulating testosterone levels. It produces this effect through inhibition of cytochrome P450 isozyme 3A4 and 17,20-lyase, which are involved in the synthesis and degradation of steroids, including the precursors of testosterone. Due to its efficacy at reducing systemic androgen levels, ketoconazole has been used as a treatment for androgen-dependent prostate cancer. Second, ketoconazole is an androgen receptor antagonist, competing with androgens such as testosterone anddihydrotestosterone (DHT) for androgen receptor binding. This effect is thought to be quite weak, even with high oral doses of ketoconazole.

When it is not to be taken (Contraindications):

Contraindicated in patients who are taking terfenadine or astemizole, cisapride, triazolam and hypersensitivity.

Category C: Animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in humans, but potential benefits may warrant use of the drug in pregnant women despite potential risks.

Dosage & When it is to be taken:

Adult- The oral dose range is 200-400 mg daily. Topical formulations are administered over affected areas once or twice daily.

How it should be taken:

It comes as a tablet to take by mouth, with or without food. It also comes as a cream to apply topically. Warnings and Precautions:

Caution should be exercised in patients with history of liver disease, alcoholism, blood problems (eg, prophyria), any allergy, who are taking other medications, during pregnancy and breastfeeding. It may cause dizziness or drowsiness, do not drive a car or operate machinery while taking this medication. It may affect blood sugar level; monitor sugar level regularly.

Side Effects:

Headache, dizziness, drowsiness, fever, chills, sensitivity to light, diarrhea, nausea, vomiting, breast enlargement in male, impotence, decrease in platelet counts and white blood cells, anemia, projection of skull bone, hair loss.

Other Precautions: Avoid excess dosage.

Storage Conditions: Store it at room temperature. Protect from heat, light and moisture. Keep away from reach of children.
