Rapamune (sirolimus) is an immunosuppressant agent developed to reduce organ rejection in patients receiving kidney transplants.

14 April 2014, Texas, USA, (http://daynighthealthcare.com/) : Rapamune (sirolimus) is an immunosuppressant agent developed to reduce organ rejection in patients receiving kidney transplants. It is to be used in combination with cyclosporine and corticosteroids, and has been shown to significantly reduce rejection rates when compared to control regimens.

Kidney transplantation is one of the most common transplant procedures performed in the United States. Patients receive immunosuppressant agents in order to lower the body's normal immune response, which in turn decreases the risk of organ rejection by the immune system.

When it is to be taken:

-   Adult: PO- Low-moderate risk patients: 6 mg on day 1. Maintenance: 2 mg once daily, given 4 hour after ciclosporin.
-  High immunological risk patients: Recommended loading dose: 15 mg. Maintenance: 5 mg/day.

How it should be taken: It comes as a tablet to take by mouth, on an empty stomach or with food.

Special Instruction:

-  Caution should be exercised in patients with history of liver, kidney problems, high cholesterol, low blood platelet levels, diabetes, lung or breathing problems, any allergy, infections, who are taking other medicationsespecially for HIV or Hepatitis C, during pregnancy and breastfeeding.
-  Avoid contact with people who have infections.
-  Patient may develop with increased risk of progressive multifocal leukoencephalopathy (PML), severe kidney problems, certain types of cancer; if it is so consult with your doctor.
-  Avoid live vaccination while taking this medication.
-  It may affect blood sugar level; monitor sugar level regularly while taking this medication.

Mechanism of action

Sirolimus inhibits T lymphocyte activation and proliferation, which occurs in response to antigenic and cytokine stimulation; however, its mechanism is distinct from that of other immunosuppressants. It also inhibits antibody production. In cells, it binds to the immunophilin, FK Binding Protein-12 (FKBP-12), to generate an immunosuppressive complex. The sirolimus: FKBP-12 complex has no effect on calcineurin activity. This complex binds to and inhibits the activation of the mammalian Target of Rapamycin (mTOR), a key regulatory kinase. This inhibition suppresses cytokine-driven T-cell proliferation, inhibiting the phase progression of the cell cycle. What should I discuss with my doctor before taking sirolimus?

Sirolimus may increase your risk of developing lymphoma or other forms of cancer. Talk with your doctor about your specific cancer risk. You should not use this medication if you are allergic to sirolimus.

To make sure you can safely take sirolimus, tell your doctor if you have any of these other conditions:

-  high cholesterol or triglycerides;
-  cytomegalovirus (CMV);
-  liver disease; or
-  Kidney disease.

Side Effects:

-  Most Common - Swelling in the extremities, high blood pressure, increased cholesterol level, constipation, abdominal pain, diarrhea, headache, fever, urinary tract infection, anemia, nausea, joint pain, and decrease in platelet counts.
-  Body as a Whole - Sepsis and infections.
-  Heart- Blood clot events and fast heart rate.
-  Gastrointestinal - Mouth ulcer.
-  Metabolic - Abnormal healing increased level of lactic dehydrogenase (LDH) and decrease in blood potassium level.
-  Musculoskeletal - Death of bone cells.
-  Respiratory - Nose bleed and lung infection.
-  Skin - Skin cancer and rash.
-  Genitourinary - Kidney inflammation

Other Precautions: Avoid excess dosage.

Storage Conditions: Store it at room temperature (25°C), and in an airtight container.
